Policy & Consent -Smiles Against Bullying

Participation in the Smiles Against Bullying campaign is voluntary. It does not imply endorsement or recommendation by any school or school district, including but not limited to Broward or Miami-Dade County Public Schools.
Smarter Orthodontics by Dr. Maria Yazji reserves the right to select recipients based on program criteria and professional evaluation.
The final selection is made solely by the Smarter Orthodontics team.
Parent or legal guardian consent will be required before any orthodontic treatment is provided.
By submitting this form, you authorize Smarter Orthodontics to review the case and, if selected and with parental consent, to share elements of the student’s story (such as before/after photos and testimonials) for educational and promotional purposes related to the campaign.
No costs will be incurred by the school, nominee, or their family for the treatment provided under this initiative.
