By submitting a nomination, you acknowledge the following:

• Participation in the Smiles Against Bullying campaign is voluntary. It does not imply endorsement or recommendation by any school or school district, including but not limited to Broward or Miami-Dade County Public Schools.
• Smarter Orthodontics by Dr. Maria Yazji reserves the right to select recipients based on program criteria and professional evaluation.
• The final selection is made solely by the Smarter Orthodontics team.
• Parent or legal guardian consent will be required before any orthodontic treatment is provided.
• By submitting this form, you authorize Smarter Orthodontics to review the case and, if selected and with parental consent, to share elements of the student’s story (such as before/after photos and testimonials) for educational and promotional purposes related to the campaign.
• No costs will be incurred by the school, nominee, or their family for the treatment provided under this initiative.
